Request a Registration Number Reset

To get your registration number reset for reactivation, please follow these simple steps:

1. Fill out this form as completely as possible. At minimum, you must fill in the required fields.
2. Review the policies prior to submitting your information.
3. You will see an confirmation of your request immediately in your browser.
4. You should hear back from us within the next business day once we receive your request.

General Reset Policies

• Apteryx has the right to request further information and necessary documentation prior to a reset.
• Administrative fees may be invoked for abuse of reset privileges.
• Invalid or fraudulent information will render all requests for that registration number to be automatically and permanently denied.
• Registration numbers previously activated for a generic user (a, Dr., etc.) will not be accepted for reset if there is no other valid information on file with Apteryx for that user.
• If a registration number is reset for any reason and ends up registered to another user/office/network, it will be automatically billed to your company.
• Apteryx reserves the right to amend these policies at any time.
